Release of Information requests:

There are certain circumstances, fortunately rare, where mental health/therapy records are needed to fulfill a referral, insurance, or legal situation. Releasing records should never be taken lightly, as they are highly private and should remain so. Once released, often the recipient of the records has no confidentiality requirement. I have no control over how the record is used once it is released. Consistent with professional standards, HIPAA, federal and Texas state law, mental health records are destroyed after 7 years from last therapeutic contact/termination. There will be no records after 7 years. This standard of the field is intended to represent the balanced interest of clients.

For those few who may need me to send records to another therapist or other bonified recipient, or have questions about your records, you can get in touch with me. I will continue to have my fax number and phone available. See my updated contact information as needed. Any release of records requires the client’s written consent, with the exceptions of a legal subpoena or an emergency that ethically or legally requires my release.
